I do not see the characters with the same code name as a the same person. Never have. I look at their actual birth name on the file-card. Here is a complete list of potential problems within the code names that many may have:
• Airborne [Franklin E. Talltree] • Airborne [Robert M. Six] • Breaker [Alvin "Breaker" Kibbey] • Breaker [Abel "Breaker" Shaz] • Cobra Commander [Unknown] • Cobra Commander [Rex “The Doctor” Lewis] • Dial-Tone [Jack S. Morelli] • Dial-Tone [Jill S. Morelli] • Doc [Carl W. Greer] • Doc [Carla P. Greer] • Heavy Duty [Lamont A. Morris] • Heavy Duty [Hershel Dalton] • Rollbar [Robert Dube] • Rollbar [Reed Williston] -The thing I do not like is when they change the race and then designate then with the same code name and birth name. For Example: Ripcord [Wallace A. Weems]. -The other thing that I do not like is when they give a character with the same birth name many different code names (Why would they add their rank in their Code Name is beyond me too): • [Daina L. Janack] Diana/Volga • [Franklin E. Talltree] Ripcord/Air Raid • [Sherman R. Guderian] Heavy Metal/Rampage/Speed Metal • [Matthey Harris Breckenridge] Thunder/Sgt. Thunderblast • [Clayton M. Abernathy] Hawk/Tomahawk/General Hawk • [Craig S. McConnel] Rock n Roll/Bench Press • [Wallace A. Weems] Ripcord/Spc. Altitude • [Blaine L. Parker] Main-Frame/Data-Frame • [Jason A. SPC Altitude, the 25th, had the file name of Freefall, Philip Arant (or something like that), no Wallace Weems.
The original name of Altitude was part of Sky Patrol. So the 25th SPC Altitude had the name of one character, look of another character, and filecard of a 3rd character. You missed Lifeline: Edwin Steen/??? Scott The code-names from the past to now (Mainframe/Data Frame - Shockwave/Shockblast) is because of copyright issues.

One of my biggest pet peeves is when the code-name is an actual billet/ASI/SQI that's unrelated to what the Joe actually does. Granted, there's only one Joe that I know of that's a really major offender when it comes to this and that's Pathfinder. I know they probably used the word pathfinder in the colloquial sense (as in "a trail guide," which I suppose is conceptually related to his job as a "Jungle Assault Specialist") but in the military, "pathfinder" refers to a very specific job: these are paratroopers tasked with landing zone/drop zone preparation and providing forward navigational aid to pilots. But his filecard says nothing about him being airborne-qualified, his being a graduate of the Pathfinder School, or that he does any actual pathfinding (airborne infantry support of tactical aviation operations) at all. To me, it's the equivalent of giving a character the code-name "Ranger" and then the character's filecard making no mention of him attending Ranger School and/or serving with the 75th Ranger Regiment, and then it turning out that he's, I dunno, a tank commander or artillery crew member or something radically different from what his code-name suggests.

It has always peeved me that--
Code names are supposed to be super-top-secret callsigns used for missions so that you're not referring to highly sensitive agents in highly sensitive parts of the world on highly sensitive operation by their real names. Yet Vincent R. Falcone's codename is "Falcon." That's not very creative or deceptive. And he's the only Joe (that I know of) on the team with a codename the same as his actual name. Might as well have been calling him "Vinny" all along. You figure, a guy like that would probably have grown up his entire life with the nickname "Falcon," so everyone that ever knew him growing up would know him by that name anyway!

Not really a pet peeve but more of a "dumb idea for a codename" is the use of the codename "Mayday" (a character from GI Joe Extreme who also appeared in the DDP comics). I could imagine it being confusing as hell for the radio operator when she's in the command post and there's an emergency. Is the person on the horn calling for her? Are they sending out a distress signal? It's like a set-up for a comedy sketch.

My pet peeve was when they put Sgt. in front of everyone's names.
