As a horror film nut (I even recently published a role-playing game based on them called Slasher Flick), very few movies scare me or even creep me out much. The following films are the rare exceptions:
Black Christmas: The remake was decent, but the original masterpiece was disturbing and suspenseful in ways I can't even explain. Ya gotta love Bob Clark. Session 9: I'm actually shocked that this thread has gone on so long without even a mention of this creepy-as-hell mind-twister being listed. Halloween: Need I really explain? The visage of Michael Myers wearing his stark white mask while shrouded in shadows is something I'll never forget. Dead Silence: I didn't expect much going into this movie. Boy, was I pleasantly surprised. This movie exuded creepiness.

and i watched "irreversible" once. the music in the beginning is made to make the audience feel uneasy, it made me sick. not to mention the awful 9 minute long rape scene. made my stomach turn for days.

I'm not going to read nine pages just to see if the George C. Scott classic the Changeling has been mentioned...
or Serpent and the Rainbow...

The most scared I have ever been watching a movie was Communion when I was a kid. There is this scene when I think Christopher Walken wakes up and he senses something wrong and they show a little gray alien with his creepy eyes peeping out from behind a door. I was real big into aliens and ufos back then. LOL.

PHANTASM still has a profoundly chilling feel to it.
ALIEN gets to me even now, despite me knowing it back to front. EVENT HORIZON also had a "something bad is going to happen" vibe but that's it really. I get my 'chills' from old television. BBC 'ghost stories' are excellent examples of atmosphere over gore & cheap 'boo' scares and they come highly recommended from me. A WARNING TO THE CURIOUS, THE STONE TAPE and THE SIGNALMAN are all very scary in the proper sense of the word.

The scariest movie is "Audition". If that movie doesn't scare you, then nothing will.
