I'm a little sensitive to the issue of missed CTs lately. Week and a half ago I'm doing a consult on a patient who's on a hospitality ward (that's basically a family doctor who has beds in hospital).
During the discussion, this patient turns out to have had symptoms classic for an intractanial bleed. However, because there's also nausea/vomiting with a recent history of exotic travel, it's dismissed as some form of stomach flu. Nausea/vomiting in the setting of this patients other symptoms is classic for blood in the head. And no one notices this till a relative of hers (who's also a doc) flies in from several thousand kilometres away to convince them that's what's going on. Guess what the CT shows? Blood at the base of the brain. We never did figure out why it happened, since it was a week later. Which of course means we have no way to predict/prevent it occurring again. Also worthy of note, if it had been an aneurysm (that, we could rule out thankfully) there would have been a significant chance of fatal rebleeding in the week this went unchecked. All because someone didn't order a scan. So, if you ever have weird headaches, or get hit in the head, get a damn CT. Every sports team has an in house doc. They should know better. Risking lives right there with negligence. (Note, I recognize there's a rather significant cost barrier to the average American compared to Canadian health care in getting anything like that done. However, in this case, I'm fairly sure that multi-millionaires can afford it) |
